Overview

Cortexin is a peptide-based neurobioregulator derived from bovine brain extracts, widely used in Eastern Europe for cognitive decline, stroke recovery, and neurodevelopmental disorders. It exerts neuroprotective, metabolic, and anti-inflammatory effects, enhancing memory, learning, and synaptic repair.

Scientific findings

This multicenter prospective double-blind placebo-controlled trial randomized 272 patients with acute ischemic stroke to two 10-day courses of cortexin (10 mg IM three times daily), one course followed by placebo, or two placebo courses over 70 days. The authors reported greater recovery of neurological function and cognitive indices in the dual-course cortexin group versus placebo, with a favorable safety profile. This is among the few placebo-controlled randomized trials of cortexin, though it is manufacturer-associated and published only in Russian, limiting independent scrutiny.

Plain English

This is the strongest human trial available: 272 stroke patients were randomly assigned to receive either cortexin or a dummy (placebo) injection, and neither patients nor doctors knew which they got. Patients who received two full courses of cortexin appeared to recover better than those on placebo, with no notable safety problems. However, the study was run in Russia, published only in Russian, and connected to the drug's makers, so the findings should be viewed with caution.

Scientific findings

This preclinical study compared the neuroprotective activity of Cortexin, Cerebrolysin and Actovegin in rat models of acute (middle cerebral artery occlusion) and chronic (common carotid artery stenosis) brain ischemia over 10 days, using behavioral testing, neuroimaging, and antioxidant assays. Cortexin and Cerebrolysin, and to a lesser degree Actovegin, improved recovery of neurological function and reduced sensorimotor and cognitive deficits. Cortexin showed measurable neurotransmitter-receptor binding and evidence of blood-brain-barrier penetration. Being an English-language, peer-reviewed animal study, it provides mechanistic support but no direct evidence of clinical benefit in humans.

In rats with induced strokes or reduced brain blood flow, cortexin (and a similar drug, Cerebrolysin) helped the animals recover movement and thinking abilities better than an untreated comparison, and the drug appeared to reach the brain and interact with brain-cell receptors. This is an English-language, independently published animal study, so it is more transparent than most cortexin research. But results in rats do not prove the drug works the same way in people.

Scientific findings

This open-label multicenter screening study assessed cortexin (10 mg daily for 10 days) in 500 patients (mean age 63.7 years) with stage II chronic brain ischemia, using neurological scales, cognitive tests (Five Words, Schulte, MMSE), and mood scales (Hamilton anxiety, Geriatric Depression Scale). The authors reported reduction or regression of focal neurological symptoms, improved cognition, and reduced anxiety and depression after treatment. Because the design is open-label with no placebo or control arm, the results are highly susceptible to bias and cannot establish efficacy.

Plain English

In this study, 500 people with chronic reduced brain blood flow were given cortexin for 10 days, and afterward their reported symptoms, memory, and mood had improved. However, everyone knew they were getting the drug and there was no comparison group taking a placebo, so improvements could easily be due to expectation or natural recovery rather than the drug itself. It is weak evidence on its own.
